Does UnitedHealthcare Cover Rehab in Georgia? Using UnitedHealthcare at Promises Atlanta

UnitedHealthcare is the largest health insurer in the country, and its family of brands covers a significant share of Georgians—often under names that do not say “UnitedHealthcare” on the card. Promises Atlanta’s UnitedHealthcare commercial contract covers substance use and mental health treatment for members of UHC, Optum and United Behavioral Health, UMR, Oxford, All Savers, Golden Rule, and Oscar Health, as well as UnitedHealthcare’s Georgia marketplace (exchange) plans. The information below describes how UnitedHealthcare plans generally work. Your plan’s terms control.

Which Card Do You Have?

UnitedHealthcare’s brands serve different markets. All are covered under our contract:

  • UnitedHealthcare (UHC) — employer group PPO, POS, HMO, and Choice/Choice Plus plans. The most common card we see.
  • UMR — a UnitedHealthcare company that administers self-funded employer plans. Your employer funds the plan; UMR processes claims. Benefits work the same way for you.
  • Optum / United Behavioral Health (UBH) — the behavioral health administrator for most UnitedHealthcare plans. If your card lists a separate mental health number, it is usually Optum.
  • Oxford Health Plans — UnitedHealthcare’s brand in New York, New Jersey, and Connecticut. Relevant if you are coming to Atlanta for treatment from the Northeast.
  • All Savers — level-funded plans for small employers.
  • Golden Rule — individual and short-term plans. Note: short-term plans are not ACA-compliant and may exclude behavioral health; verification is essential.

Optum: Who Authorizes Behavioral Health

For nearly all UnitedHealthcare brands, behavioral health benefits are managed by Optum. Optum applies its own level-of-care guidelines to approve detox, residential, partial hospitalization, and intensive outpatient treatment. Our admissions and utilization review teams work with Optum routinely. You do not need to call Optum yourself; we handle authorization from the first call through discharge.

What UnitedHealthcare Covers at Promises Atlanta

Under the federal Mental Health Parity and Addiction Equity Act and Georgia’s 2022 Mental Health Parity Act (HB 1013), plans cannot cover substance use or mental health treatment less generously than medical care—no separate day limits, higher copays, or stricter authorization standards. What Determines Your Out-of-Pocket Cost

For an in-network facility like Promises Atlanta, what you pay is governed by four numbers on your UnitedHealthcare plan rather than by the program’s list price:

  • Deductible — what you pay before the plan pays. What you have already spent this year counts.
  • Coinsurance or copay — your share after the deductible, commonly 10–30% for inpatient care.
  • Out-of-pocket maximum — the ceiling. Once reached, the plan pays 100% of in-network covered care for the rest of the year, including step-down levels and outpatient follow-up. For most people entering residential treatment, this is the number that defines their actual cost.
  • Network status — in-network benefits apply at Promises Atlanta. Out-of-network care typically carries a higher deductible and coinsurance, or is not covered at all under HMO-style plans.

How Authorization Works

  1. Our admissions team gathers clinical information and your member details.
  2. We submit a prior authorization request to Optum documenting medical necessity.
  3. Optum approves an initial number of days; detox is often same-day.
  4. We submit clinical updates during treatment to extend authorization as appropriate.

Optum is known for active concurrent review—meaning it checks in frequently on continued medical necessity. Our utilization review team is built for this. If Optum limits coverage before our clinicians agree, we request peer-to-peer review, pursue internal appeal, and explain your right to independent external review under Georgia law.

I have Choice Plus. Do I need a referral?

No. Choice Plus and most UHC PPO products allow direct access to in-network behavioral health providers.

My employer plan is through UMR. Is anything different?

Only the phone number on the card. Benefits, authorization through Optum, and your in-network status at Promises Atlanta are the same.

Does UnitedHealthcare cover residential mental health treatment?

Yes. Our residential mental health program is covered under the Optum behavioral health benefit with full parity protections.

Does Optum cover medication-assisted treatment?

Medications such as buprenorphine and naltrexone are generally covered under the OptumRx pharmacy benefit.

I have UnitedHealthcare Medicare or Medicaid (UnitedHealthcare Community Plan).

Timing, Deductibles, and the Calendar

Two timing questions come up on nearly every insurance call. First: should I wait until I have met my deductible? No. Treatment delayed for financial optimization is treatment that often never happens, and for alcohol and benzodiazepine dependence, waiting carries medical risk. If you are close to your deductible, tell us and we will factor it into the estimate; if you are not, the out-of-pocket maximum still caps the year. Second: what happens if treatment spans January 1? Deductibles and out-of-pocket maximums reset with the plan year, so a stay that begins in December and continues into January may involve two years of cost-sharing. We flag this when it applies and, where clinically appropriate, discuss how step-down levels of care fit. Neither question should keep you from calling today.
